Note that a fresh form has been returned. Repeat the previous steps until all games have been entered.

At any time during the process you can Review the list of games that have been entered by pressing the "Review/Submit Changes" button. If a game has been entered in error or contains errors it can be delete by pressing the "Delete Changes" button. Deleted games can then be re-entered if necessary.

After all games have been entered, press the "Review/Submit Changes" button.

If a tournament game or the game is played at a Neutral site, enter site in the field below.

If the site is a school, please locate the school number(SNO) by clicking on find school. When the school is located, write down the 4 position school number and return to this screen via the browser back button. If the SNO is found, enter it in the box below followed by a space and then the name of the school, otherwise enter the site of the game in the box.

If the opponent is in your conference and this games should not count as a conference match, check this box
